Emergency Department Transfer Communication
Internal Tracking and Reporting Tool
Use the Emergency Department Transfer Communication Internal Tracking and Reporting Tool (EDTC Report) to document that all elements were communicated to the receiving hospital within 60 minutes of departure. Please note that if only part of the elements were “yes,” then the response for the question must be “no.” It is either ALL or NONE. If test and procedure results have not been sent yet, but will be sent, be sure to indicate a plan for communicating them when complete.
